written authorization signed by you authorizing the agent to make the request on your

behalf and which contains the name and contact information of you and your

authorized agent.

If we are able to verify your identity, we will cease the sharing of information with

trusted businesses described above. We will notify all third parties to whom we have

sold your information within 90 days prior to our receipt of your request that you have

exercised your right to opt-out and instruct them not to further sell your information. We

will notify you once this has been completed.
